Welcome to the West Suffolk Clinical Psychology Service

 

If you have been given this website address then you may already have an appointment booked with your Clinical Psychologist or CBT therapist. Before you meet with your psychologist/therapist there will be a few pieces of paperwork that we will ask you to complete and send to us. You can complete this paperwork using this website or we can email or post this paperwork to you on request.

 

The first document that we will ask you to complete is our contact form. This form lets us know what your preferences are for us making contact with you, how much information you want shared with other professionals and what medication, if any, you are taking. Please can you read our terms and conditions for treatment before you complete this  contact form. (If you are a parent or guardian and you have booked an assessment for your child please can you ensure that you complete paperwork on your child's behalf). Finally, please could you complete our health questionnaire. This will take approximately 10 to 20 minutes.
